RESIDENTS RIGHTS

As a Resident of a licensed care facility you can have certain rights. Those rights differ based on the type of facility you are in. Listed below are different provider types. Please click on a category below to see the corresponding resident rights.

 

GRIEVANCE PROCEDURE

If you have a problem or a complaint you may contact the Division of Licensing and Protection at 1-888-700-5330 or the Ombudsman at 1-800-889-2047

IF YOU HAVE A COMPLAINT

If you have a complaint, please speak with either the administrator of the nursing home, the director of nursing or the social service director. If you are not satisfied with the facility's response and would like outside help resolving the problem, call the Division's facility compliant line at 1-888-700-5330.

IF YOU WANT OUTSIDE HELP

If you want outside help, the Vermont Department of Disabilities, Aging and Independent Living, Division of Licensing and Protection licenses, regulates and surveys nursing homes. The Division investigates complaints of poor care or sub-standard conditions. You may direct a problem or complaint to this Division by calling 1-888-700-5330 or by writing to the Division of Licensing and Protection, 280 State Drive, HC 2 South, Waterbury, Vermont 05671-2060.

If you would like someone else outside the nursing home to help you or speak on your behalf, you may contact the Ombudsman at 1-800-889-2047. The Ombudsman will keep all information, even your name, confidential unless you give permission to use it. Ombudsman help is free of charge.
